The FLYFCL carries a $1,000,000.00 general liability insurance policy from Bollinger Insurance for all its active players, cheerleaders, coaches, and both individual team and league executive board members.  If you or your child suffers an accident or injury at an officially approved team or league function, please follow these instructions.

First, make sure your teams medical director or president completes a FLYFCL accident report and submits it to the league treasurer ASAP!

Then immediately submit a claim for all medical expenses to the company that administers your own personal or group or or health care plan(including Major Medical coverage).  If you have coverage through an HMO or similar facility, you must use those medical facilities and physicians first or the claim will not be covered under this policy.

If your not covered by or don't have health insurance or your health insurance or health care plan has paid the medical expense up to the policy limits, attach any unpaid bills and copies of payments made by your insurance policy (Explanation of Benefits) to a Bollinger "Amateur Sports Accident Proof of Loss" claim form and mail them to the address shown below.  Currently the deductible with the FLYFCL and Bollinger Insurance is $250.00 per accident.
