INSURANCE DOCUMENTS
Off-Season Insurance Requests:
1) Complete the camp/clinic form and submit it to [email protected]
2) The Executive Board will review the form, collect any required additional information, then submit it for approval from the insurance company
3) Upon approval from the insurance company, a COI will be issued and an invoice created for the additional coverage
4) The submitting club will pay the invoice to FLYFCL (due within 45 days from the date of the invoice)
In-Season Insurance Claim Submission:
1) Complete the FLYFCL accident report and the insurance claim form; email both documents to [email protected]
2) Provide any additional information requested by FLYFCL or the insurance company
3) Collect and submit and follow up paperwork from the family of the injured athlete
*Ensure the family of the injured athlete is aware there is a $250 deductible for the FLYFCL insurance in addition to their own insurance policy deductible.
